A case study is meant to show your ability to apply course concepts to the given scenario. You are expected to use theories, vocabulary, and examples to answer the questions given in the case study. Your paper should be written in clear and concise language that shows your ability to comprehend course material and relate it to problems. A case study is an APA formatted essay on a particular topic. The paper is to be 4-5 pages not including the title or reference pages. A effective case study essay must contain the following: 
Introduction 
This should not exceed 350 words and should contain a brief overview of the case study. The introduction should motivate and describe the issue being studied and summarize the main findings. 
Case Description 
This should a short overview of the case, including the role of the people and institutions involved and how the information was analyzed.  
Discussion and Evaluation 2-3 pages (See specific prompts below)
This is the body of the case study where you would answer all the questions from the case study in an essay format. 
Conclusion 
The conclusion should offer a brief recapitulation of the main points made in your case study. 
Address the following questions in the “Discussion and Evaluation” section of your paper:  
What were the most pressing issues causing Dr. Pate and his leadership team to pursue transformational change?  
In what ways did Dr. Pate and his team demonstrate transformational leadership?  
What evidence shows that St. Luke’s stakeholders demonstrated effective teamwork in pursuing transformational change?  
How can transformation leadership help to mitigate the social determinants of health at St Luke’s?  
How can the leadership team address the goals of Triple Aim effectively during the transformation process?
